Liability Waiver

I am voluntarily participating in the Give Back Bus because I want to do well for others and give back to my community.  I understand that the tasks I ultimately perform may involve significant physical activity or labor, and may be performed in environments including, but not limited to, ice/snow, construction sites, the use of tools or simple machinery, elevated workstations, etc. With this knowledge, I understand that the tasks I may perform and/or the environment in which they will be performed may present inherent safety / health risks that can that range from minor injuries to catastrophic injuries.

By participating in the Give Back Bus, I knowingly and willingly assume any and all risks that may go along with doing so. Furthermore, I hereby waive any and all claims I may have against Volunteer Fox Cities, Lamers Bus, Kobussen Bus, Big Brothers Big Sisters of East Central Wisconsin, and any activity site which I will be volunteering in connection with my participation in the Give Back Bus, and release the aforementioned entities from any and all liability in connection with my participation.

In addition, I hereby give full consent to Give Back Bus and Volunteer Fox Cities to publish, reproduce, and display, without limitation all video, electronic, or photographic materials in which I’m pictured on the day of the event, as well as any of my written and/or spoken comments taken or collected during the day of the Give Back Bus. I understand I am releasing and discharging these entities from any and all claims, now or in the future relating to these images and comments, which may be used without limitation, reservation, or compensation.
